If you decide to request an application or quote for insurance, we require you to submit minimal personal information. We collect information that includes your name, e-mail address, mailing address, telephone numbers, and date of birth. The information we gather from your insurance application request is used to begin the application process. A representative from ReliaQuote will contact you to complete your application by telephone so that additional information can be obtained. Your application will then be mailed to you for your review and signatures.
